The ability to make custom spells is one of the most powerful perks that you acquire by joining the Mages Guild. You can combine multiple effects into a single, powerful spell; you can optimize the strength of spells to minimize the Magicka cost. There are even versions of some spells that can only be accessed through the Altar of Spellmaking, making spells oblivion.
In order to begin making your own spells, you'll have to join the Mages Guild and gain access to the Arcane University. Inside, go to the Praxographical Center and approach the Altar of Spellmaking. You can only add a spell effect to your created spell if you already know it and can cast it. For instance, if you don't have a Shield spell, you can't create a new spell that has that effect. Likewise, if you can't cast a Summon Storm Atronach spell that you have, you won't be able to create a less powerful version of it to cast. Now, here's where it gets a little tricky.

There is a mod for crafting spells, otherwise you buy the spells from shops that sell them and mages guild in eahc of the cities. Sadly you cant they removed that somewhy it was one of the main Reasons why i loved Oblivion. Like Tikon said, you also need to have bought spells that have the effects you want to put in a custom spell. Are you talking about making custom spells? You have to gain access to the Arcane University and then you use the spellmaking altar to create your own spells. The power of the spells you make is limited to your skill level in the various schools of magic and you must learn a base spell with the desired effect before you can use the altar to make a custom spell with that effect. You can also use the construction set to make custom spells and even make your own powers. I made a lesser power that allows you to use Mythic Dawn Armor, a greater power for bound armor, and a bunch of other cool once a day powers.
Also, the experience you gain for casting this spell will only be in the primary school. By general usage, making spells oblivion, I mean spells that don't take a ridiculous amount of magicka to cast and that I can use on most enemies.

Custom spells can only be made at an Altar of Spellmaking. There are two within the Praxographical Center building at the Arcane University. These are only available to members of the Mages Guild who have completed all the recommendation quests in order to gain access to the University. A third altar is available within the Arch-mage's personal quarters in the Arcane University once you complete the Mages Guild quests and become Arch-mage. A fourth altar is found in Frostcrag Spire , if you have the official plugin, but you must purchase the magetallow candles and upgrade the altar before it can be used. When you activate the Altar of Spellmaking, the spellmaking window appears.
