The Joker Infection was a unique condition created by the Joker 's mutated blood that slowly turned those infected into direct copies of him. Prior to the events of Batman: Arkham City , the chances for a cure to the Titan Disease were looking slim. Joker had the doctors Hugo Strange sent him drain his infected blood and transfuse healthy blood to extend his life while also having Strange send him Mr. Freeze 's wife Nora Fries as leverage to get Freeze to develop a cure. Not wanting to go out by himself, Joker blackmailed Strange into sending his tainted blood to hospitals all over Gotham City by reminding him about his experiments with the Arkham Lunatics , Mad Hatter and Quincy Sharp. Joker even managed to poison Batman with his blood forcing the Dark Knight into helping him get the cure for the disease. Unfortunately for Joker, when Batman gained the cure and took his share the Clown Prince of Crime caused him to drop the vial which made the spilled cure lose its potency and fail to heal Joker, Batman then revealed even after all the horrible things Joker did, he would've given him the cure. Although Batman managed to replicate and distribute the cure, saving thousands of lives as well as rounding up most of Joker's contaminated blood samples with the help of James Gordon , four people were given blood transfusions with blood samples they missed due to hospital errors leaving the transfusions unrecorded. It was sometime afterwards that those individuals began showing signs of the infection leading to their discovery and incarceration by Batman. Batman linked the infection to being a form of Creutzfeldt-Jakob disease CJD , but it was mutated beyond anything on medical record. Like CJD, the infection attacked the brain and damaged the tissue affecting the victim's mood and behavior. After repeated nightmares and attempts to understand them, Batman feared the infection would take him over as he still had the mad clown's blood in him as well, keeping a fifth quarantine cell ready at Panessa Studios. Fear Toxin allows Batman's Joker personality from the infection to manifest as hallucinations. To his horror, he found Scarecrow 's new strain of Fear Toxin could exaggerate the infection to the point of causing hallucinations. In Batman's case a vivid hallucination of Joker appeared that represented the embodiment of his worst fears and darkest desires along with Scarecrow Nightmares and flashbacks to Joker's past crimes.

Later that evening, political activist Quincy Sharp made an appearance on the Jack Ryder Show to announce his intentions to reopen Gotham's infamous Arkham Asylum in an effort to create a more secure facility to rehabilitate and secure the most dangerous criminals. The dying hero then entered Clayface's body, retrieved the cure, cut him open, and incapacitated the monster once and for all. This macho posturing amused the Joker, who settled in to wait. As Ivy continued to yell about Batman killing her plants, the Joker moaned over the intercom if she ever shut up about them. However, Katie was diagnosed with an incurable disease, which left Burke devastated. He was also aware of being observed obsessively by one of the doctors while in his cell and put on a show whenever released, spontaneously brutalizing fellow inmates and asylum staff just to keep everyone on their toes. Any slip-ups and out of character activity were put down to Black Mask sampling his own product. Months later, Dr. Batman went after Harley, who sent her thugs after Batman again, though he managed to defeat them all. A secret alarm placed by Batman had alerted him to the Joker's escape, and here he was in the Batboat, pursuing the fugitives. Soon after his arrival in Gotham City, the Penguin decided to make an impression on the underworld elite with a gala opening for his nightclub, the Iceberg Lounge. Over the following weeks, as Arkham City was shut down, Batman saw to it that a massive quantity of the cure for the Titan Disease was manufactured and administered to all of Joker's victims, which cured them and saved their lives. Batman investigated the power outage, overpowered the Joker and broke his nose.
The Joker was a notorious mass murderer, terrorist and anarchist with white skin, green hair and red lips with a tendency to wear purple clothing, concealing the true force of chaos underneath his colorful appearance.
However, Joker was angered when Batman resisted and fought off his attempts to make him give in. He promptly vanished only to be found three months later with a new smile carved onto his face. Batman easily avoided the makeshift blades and continued to beat the Joker, even throttling him! Aside from the obvious adoption of Joker-like traits and to a certain extent hallucinating Joker, the disease also results in the infected having intrusive flashbacks to heinous actions Joker had caused in the past, regardless of whether the infected ever witnessed such actions. However, Joker did eventually figure out that Hugo Strange had brainwashed Sharp. Rather than fight Batman, upon his arrival and confirmation of being on the 'guest list', the goons acted friendly, cheered him on, and allowed him safe passage. In one particularly gruesome crime spree he kidnapped a number of children, returning them to their parents but as horribly parodies of themselves; he had dismembered them and sown their body parts together at random. The vigilante dealt with Deadshot before he made his way back inside and to the conference room, where he found Warden Joseph tied to explosives Joker had rigged to security junction boxes. When Batman had accomplished everything that he was blackmailed to do, Joker was delighted to begin his cat and mouse game with his old enemy and friend anew. After Batman stopped him from escaping Arkham Asylum, the Joker sat alone in his padded cell, knowing something was wrong.
